Scouts and adults under age 40:

Class 1 (PDF) - sufficient for weekend campouts and does not require a doctor's exam
• Needs to be updated every year.

Class 2 (PDF) - combines a doctor's exam to the info from the Class 1 form
• Needs to be updated within 36 months.

Class 3 (PDF) - requires a doctor's exam, and can be used instead of Class 1 and Class 2 forms
• The Class 3 form replaces the need for both the Class 1 and Class 2 forms.
• Valid for 12 months only so the doctor's exam is required every year.

• For all summer camps, at least the Class 1 & 2 combo or the Class 3 forms are required.
Troop 55 prefers the Class 3 form for summer camp.

• Scouts going to a High Adventure summer program (not regular summer camp) are required to use Class 3.

Adults over age 40: must use the Class 3 Medical Form (PDF)
• The Class 3 form is valid for 12 months only so the doctor's exam is required every year.

Waiver and Consent to Treat form - (PDF) or (Word format)
• These forms are required for all scouts and adults.
